Deep cerebral venous thrombosis is a rare disorder involving the internal cerebral veins with or without cortical vein or dural venous sinus thrombosis. A 29 year old man was presented with squeezing and pressing type headache. Neurologic examination revealed subtle neck stiffness. Brain MRI showed high signal intensity lesion in the left thalamus. Multifocal filling defects in the straight sinus, vein of Galen, and left internal cerebral vein were confirmed by thin section sagittal T1WI and 3D CE-T1WI, which suggesting multifocal venous thrombosis. He was treated with heparin and the symptoms disappeared.
